Treatment Plan Estimate

Once we have assessed your dental condition, we will present you with a treatment plan. Our team will provide a clear breakdown of the recommended treatment, the estimated insurance benefit (if applicable), and your expected out-of-pocket responsibility. We’re happy to answer any questions about fees and discuss your options.

Please note that dental benefits are subject to various limitations as determined by your benefits provider.

Co-payments

Payment is due at the time of service. If you have dental insurance, your estimated co-payment is due at the time of services. We accept payments in the form of cash, bank debit cards, Visa, Mastercard, American Express, and Discover.

Dental Insurance

We accept most PPO dental insurance plans and will complete and submit the dental claim form as a courtesy to you. Our front office team will verify your benefits before your appointment and provide an estimate of your out-of-pocket costs so there are no surprises.

Your insurance policy is a contract between you and your insurance company. As health care providers, we are not a party to that agreement. If your insurance does not cover or only partially covers the cost of the treatment provided, you will be responsible for the payment of any remaining fees. This holds true regardless of the estimate initially provided to you.

If you are covered by a plan that gives you a discounted fee schedule, the discounted fees will be applied to your account and recognized by the insurance carrier upon receipt of your claim. If your insurance has a preferred provider list and we are not on that list, you may be responsible for additional costs.

We are committed to helping you maximize your benefits and will work with you to achieve the maximum coverage available under your plan.

Dental insurance can be intimidating and confusing. Should you have any questions, please do not hesitate to ask for clarification before your appointment.

Payment Plans

As a courtesy to our patients, we offer payment plans through CareCredit. CareCredit offers flexible monthly payment plans—many with promotional interest-free periods—so you can get the care you need now and pay over time. You may apply online, by telephone, or by facsimile.

Late Fees & Account Balances

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Most insurance companies will respond within four to six weeks after a claim is submitted. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility, and your prompt remittance is appreciated.

Should your account exceed sixty (60) days, one and one-half percent (1.5%) interest per month (18% per year) will be charged. In the event your account exceeds ninety (90) days after all insurance claims have been paid, your account will be sent to a collection agency and/or small claims court, and an additional $85.00 will be charged for administrative fees.
