We are committed to providing excellence in endodontic care for our patients. Included is a commitment to open and clear financial policies.
Treatment Plan Estimate
Once we have assessed your dental condition, we will present you with a treatment plan. Please note that the dental benefits are subject to various limitations as determined by your benefits provider.
Co-payments
Payment is due at the time of service. If you have dental insurance, your estimated co-payment is due at the time of services. We accept payments in the form of cash, bank debit cards, Visa cards and Mastercards.
Dental Insurance
Your insurance policy is a contract between you and your insurance company. As health care providers, we are not a party to that agreement. If your insurance does not cover or only partially covers the cost of the treatment provided, you will be responsible for the payment of any remaining fees. This holds true regardless of the estimate initially provided to you.
Ultimately, we are committed to helping you maximize your benefits and we will work with you to achieve the maximum benefits for your coverage. If you have dental insurance, we will complete and submit the dental claim form as a courtesy to you. If you are covered by a plan that gives you a discounted fee schedule, the discounted fees will be applied to your account and recognized by the insurance carrier upon receipt of your claim. If your insurance has a preferred provider list and we are not on that list, you may be responsible for additional costs.
Dental insurance can be intimidating and confusing. Should you have any questions, please do not hesitate to ask to clarify before your appointment.
Payment Plans
As a courtesy to our patients we offer payment plan through CareCredit. You may apply online, by telephone, or by facsimile.
Late Fees
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks after the claim is submitted. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
Should your account exceed sixty (60) days, one and one-half percent (1.5%) interest per month (18% per year) will be charged. In the event your account exceeds ninety (90) days after all insurance claims have been paid, your account will be sent to a collection agency and/or small claims court and an additional $85.00 will be charged for administrative fees.
